Creating a Comprehensive Care Plan

A Comprehensive Care Plan can be customized to meet each individual’s goals and need. Families often benefit from a thorough care plan which helps guide them in the right direction. Once a care plan is created, we work together to facilitate the next step in your loved one’s care. 

  • Assess the client’s medical, functional ability, psychological/social and living conditions to identify care and safety needs.
  • Serve as liaison and advocate – between physicians, financial managers, attorneys, and family members to make certain the client’s overall needs are met.
  • Coordinate communication through emails and phone calls.
  •  Individualized plan of care – designed to develop the client’s overall objectives and care concerns detailing the outlines of a person’s ADL’s and IADL’s specifically designed to each
    person’s needs.
  • Facilitate family meetings and/or meetings in a neutral and unbiased manner with key individuals determined by the client who are to have details of their health care mapping. 
  • Planning and executing your personal outline – after a trust or will has been established with an attorney. Allowing a fluid communication between one’s physicians, attorneys, home health agencies and care companions agencies. This navigational
    outline provides a mapping of the details of who, what and where. This is advantageous not only for the trusts (trustees) but is important if a catastrophic event occurs and information is
    needed to be obtained.
  • Visit clients on site – to supervise their continued care, set up and reorder medications and other household items needed to support their in-home environment. 
  • Medication outlines – The disconnect between Hospital, Physician, Rehabilitation etc. allows mismanagement of information leading many times to confusion by the client as
    well as family members. We can streamline making communication and verification fluid for one’s healthcare pathways.
  • Long term care (LTC) insurance policies – if you or your loved one needs assistance with ADL’s or IADL’s, we can work with your LTC (Long Term Care Insurance) carrier to get you
    qualified and approved for service. 
    • ­Consulting set-up establishing the claim eligibility
    • ­Initiating the claim- outlying directives
    • ­Claim Form – collection and review
    • ­Submission of claim to carrier
    • Follow-up – additional submissions (if necessary) verifications, initial review/claim reimbursement and monthly claim reimbursement

Next Steps

Initial Assessment is vital information for planning care strategies which include short term and long-term goals. This allows us to gain insight into the medical, social, financial, and legal concerns and help you problem-solve. Our relationship with you and your loved one involves creating a short-term and long-term care plan. Every situation is unique and there are many facets to planning your loved ones care. The initial process is (1-2 hours in the home/facility) which includes the following:

  • Assessment of activities of daily living (ADL) and instrumental activities of daily living (IADL)
  • Home safety concerns – i.e.; risk of falling, home hazards and improvements. Home modification issues
  • Memory loss
  • Medical status/need – what is currently needed and anticipated, as well as, one’s medical history
  • Cognitive function and judgment
  • Emotional, social and spiritual issues
  • Mental health concerns
  • Nutrition
  • Support system for client – network of family/friends
  • Legal issues and financial issues
  • Psychological issues

  • Provide telephone and/or email updates to DPOH/DPOA, Trustees, Family Members designated by client through a HIPPA authorization

Care manager services may also include daily or weekly phone calls, coordination of doctor’s visits as well as working with the coordination of a care agency and other support services. Ongoing monitoring of the client’s mental and physical status is often important as busy and long-distance family members depend upon the dissemination of timely information to monitor the progress of their loved ones who wish to remain in the comfort of their own home but are needing assistance to maintain their independence. A long-term care plan is a great tool for the family/trustees to use as a guide for what may happen in the near future, managing an immediate crisis, prevention of crisis, and includes a plan of action giving the family a “healthcare pathway” on how they might be better prepared (emotionally and physically) for upcoming changes that may occur in the aging process.

BRENDA ORNELLAS

Brenda Ornellas is a Gerontologist with a master’s degree from the University of Southern California and has over 30 years of non-medical custodial care experience. Her wide range of experience comes from managing all aspects of a home care organization, including human resources, marketing, company oversight, care management, working with hospice, long term care insurance programs and brings the values essential to the non-medical industry. Brenda serves as the Vice Chair of the American Board of Home Care’s San Diego’s Chapter, and active Member of the Gerontological Society of America, American Society on Aging and has been an advocate for the seniors since 1993.

Additional qualifications include: Certified Matter of Balance Trainer, is criminal background checked, bonded and insured, carries Workers Compensation Insurance as well a Liability Insurance. She brings insight and vision for an innovative future for the home care industry. Brenda is registered with the Department of Social Services / Home Care Services Bureau and HCA and owns Cura Home Health Inc. which is also registered as an HCO (Home Care Organization) with the State of California Department of Social Services.

Born and raised in Southern California Eleanor has over 25 years of experience in the Home Care Industry She brings a depth of knowledge, passion for those in the aged community and a depth of practical application in helping families with the processes of Long Term Care Policies (LTC). Additionally, Eleanor is a care management and professional liaison. Her hobbies are many but her passion lies in her community service. She has served as President/Director of various PTO’s in Point Loma, a member of Soroptimist International, as well as a volunteer in her church.
